By submitting this request, I acknowledge that the information provided is accurate to the best of my knowledge; the name, address and organization name, if applicable, match the records of my current provider; I am the customer or an authorized representative of the customer; I am at least 18 years of age; and I must keep all numbers being transferred active with my current carrier until the port completes to avoid delays or rejection.
